Disease Management
The Disease Management Programs are administered as an integral part of the CLINIX Case Management program. Registered Nurses with clinical expertise in disease management will coordinate the clinical care needs and program participation of members. Education, self-monitoring, medical resource access, and community information are focal points of these programs.

- Various other chronic conditions
They are designed to educate and influence member’s behaviors related to self-care; giving patients the support and educational materials they need to better manage their disease. These programs focus on the importance of positive attitudes toward self care and management.
In addition to improving attitudes towards self-care and overall good health, the primary goals are better outcomes of care, better measurable processes of care, less morbidity, less rescue care, and lower overall costs of care.
